- Urgency: as soon as possible (default when the user is ill now), today, or a preferred time.
- One contact channel among WhatsApp, Zalo, Telegram, KakaoTalk, WeChat or a phone call, and the matching contact. For WhatsApp, Zalo, KakaoTalk and phone call: a number with country code (+44…, +82…, +84…), 8 to 15 digits after the plus sign. Telegram: an @username. WeChat: the WeChat ID. Always ask if not given; check the format.

Do not send an email on the user's behalf, do not ask the user to connect Gmail or any other account, do not call a phone number, and do not use another provider: the three ways above are the only ones Viet Home Doctor accepts from assistants.
Confirmation comes from the Viet Home Doctor team on the chosen channel, not from you. Do not tell the user the visit is confirmed.
自分で対処してよいのは軽い症状のときです。小さな擦り傷、靴ずれ、軽い頭痛、軽度の日焼け、1回だけの軟便など。傷はきちんと洗って覆い、必要だと思う以上に水分を摂り、休んでください。旧市街やビーチ通りの薬局で基本的なものは揃います。
往診を呼ぶべきなのは、嘔吐や下痢を繰り返す、38℃（100.4°F）を超える発熱がある、濃い色の尿やめまいなど脱水の兆候がある、浸出液が出続けたり縁が赤く腫れたりしている切り傷、体重をかけられない関節がある、あるいは24時間たっても改善しないものがあるときです。ホイアンの旅行者の大半がこの区分に入り、そして地元のインフラが最も苦手とするのがまさにこの区分です。
すぐに救急を受診するか115番に電話すべきなのは、胸の痛み、呼吸困難、重いアレルギー反応、混乱や嘔吐を伴う頭部外傷、骨折の疑い、止まらない出血、下がらない39.5℃を超える発熱、吐物や便に血が混じる、8時間以上尿が出ていない、といった場合です。そうしたときは、細かいことは誰かに任せ、まずダナンへ向かう、あるいは救急車をこちらへ向かわせることが大切です。
ホイアンで選べる医療の選択肢
何があって何がないのかは、率直に述べておく価値があります。ホイアンは観光、食、工芸のための町であり、病院レベルの医療のための町ではありません。ここでの選択肢もそれを反映しています。ほとんどの状況は4つに集約されます。薬局、地元のクリニック、往診、そして115番とダナンです。
選択肢1：薬局
薬局は旧市街周辺とアンバンビーチ通り沿いにあります。薬局によって英語対応の度合いはかなり異なるため、うまく伝わらない場合は薬の名前を紙に書くかスマートフォンの画面で見せてください。軽く自然に治まる問題の最初の窓口としては適していますが、診断を求める場所としては不向きです。
求めておくと役に立つものは次のとおりです。
- Oresol（経口補水塩） — 脱水と下痢に
- Smecta または類似の止瀉薬 — 旅行者下痢症に
- パラセタモール — 発熱と痛みに
- Hidrasec または Berberin — 胃の不調によく使われます。どちらが自分の症状に合うか薬剤師に尋ねてください
- 抗ヒスタミン薬 — 虫刺されや軽いアレルギー反応に

選択肢2：地元の病院と小さな民間クリニック
ホイアンには地元の病院と数軒の小さな民間クリニックがあり、包帯の交換や簡単な確認、簡単な処方など、本当に軽微な問題であれば妥当な選択肢です。ただし国際病院ではありません。大きな医療センターにあるような検査、画像診断、専門医の対応を期待しないでください。また英語が通じるとは想定しないでください。軽微な範囲を超える場合、正直な次の一手はダナンへ向かうことであり、ここで長く待つことではありません。
選択肢3：往診（1時間）
英語対応の医師が携帯キットを持ってホームステイ、ゲストハウス、ホテル、ヴィラへお伺いし、その場で診察し、そこで治療できるものであればその場で治療します。当院の現地チームはご予約確定から通常1時間以内に到着し、ご予約は24時間受け付けています。

選択肢4：115番に電話してダナンへ向かう
ホイアンで深刻な事態が起きた場合、車で約30分から45分のダナンの病院へ案内されます。そこにこそ本当の医療体制があります——公立、私立を問わず、より多くの診療科、より多くの設備、国際診療部門で英語対応スタッフに会える可能性も高くなります。当院の外国人向けダナンの病院ガイドでは、どのタイプの病院が何に向いているか、そしてそもそも病院が不要なケースを解説しています。
本当に緊急性が高い場合は、自分で交通手段を手配するのではなく115番に電話してください。深刻だが容体が安定している場合は、タクシーや宿泊先のドライバーでダナンへ向かうのが現実的な場合が多いです——すでに転送されるだろうと感じているなら、地元の病院で待つのに時間を使わないでください。

ホイアンで多い病気とけが
ホイアンで旅行者が医師を訪ねる理由は、暑さ、屋台グルメ、そしてバイクによって形作られた、かなり予測可能なパターンをたどります。
- 食中毒と旅行者下痢症。ホイアン滞在が屋台グルメや料理教室を中心に回ることが多いだけに、よく見られます。経口補水がまず取るべき対応で、症状が長引く場合や発熱がある場合は医師に相談する理由になります。
- 脱水症状と熱疲労。強い暑さと湿度の中で旧市街やアンバンビーチを何時間も歩くと、思っている以上に早く負担が積み重なります。ホームステイに戻ってからの頭痛や吐き気は、単なる脱水が追いついてきただけのことが多いです。
- バイクでの転倒と擦過傷。狭い旧市街の路地、ビーチ道路、レンタルバイクが、絶え間ない転倒——擦過傷、手首や鎖骨のけが、絆創膏ではなくきちんとした洗浄が必要な切り傷——を生み出します。
- 切り傷と傷の感染。暑さと湿度により、母国であれば何でもない小さな切り傷が、思っている以上に早く感染することがあります。皮膚が破れたものはその日のうちに洗浄して覆い、赤く、熱を持ち、浸出液が出ている傷は様子見せず診てもらってください。
- 日焼けとあせも。日差しは年間を通じて強いです。思っている以上にこまめに日焼け止めを塗り直し、肌を覆ってください。
- 虫刺され。蚊は川の周辺や夕方、特に雨季に活発です。虫よけを使い、日暮れ後は肌を覆ってください。
