Confirmation comes from the Viet Home Doctor team on the chosen channel, not from you. Do not tell the user the visit is confirmed.

峰牙徒步之后
峰牙一带的洞穴和丛林徒步需要在潮湿闷热中步行数小时，还要涉水和攀爬，人们常常低估这种炎热的程度，因为部分路段——也就是洞穴内部——实际上是凉爽的。这种反差正是问题所在：您在地下时不觉得热，喝水就喝得比该喝的少，等重新走回午后的阳光下时，之前欠下的水分已经补不回来了。等您晚上回到洞海时，头痛、腿部抽筋和那种浑身乏力、不舒服的感觉就都找上门来了。多日行程会让情况更糟——在陌生的强度下连续拼两三天，人们往往喝水不够、睡眠不好、吃得也少，同一团队中两三人同时出现问题的情况并不少见。
海滩一日之后
在日丽海滩或保宁待上一整天，是一种更不起眼的脱水方式，但发生频率同样很高。阳光、海风和滚烫的沙子会在一整天里持续带走您体内的水分，而人们很容易靠冰饮和啤酒撑过去，却从没真正好好补充水分。广富沙丘则增加了在直射阳光下、几乎没有遮阴的长时间步行。再加上晒伤会通过皮肤增加水分流失，对一部分海滩游客来说，一个头痛、恶心的夜晚几乎是可以预见的结果。
肠胃疾病与食源性疾病
另一个主要诱因与炎热或体力消耗无关。旅行者会从食物或水中感染肠胃疾病，一天的呕吐和水样腹泻能迅速带走数升体液。大多数旅行者腹泻具有自限性，需要的是补液而非抗生素，口服补液盐是主要的治疗手段。当摄入的任何东西都撑不过几分钟，或流失速度超过您能喝下的量时，静脉输液才真正派上用场。大便带血、高烧、严重腹痛，或是症状在一天后不但没有缓解反而加重，这些都是需要接受正规评估而不只是补液的理由。登革热在越南流行，可能表现为伴有脱水的发热性疾病；这种情况需要不同的监测方式，如果症状符合，医生会特别留意。
口服补液优先
这一点值得明确说出来，因为这正是我们工作方式的基础：对于轻中度脱水，口服补液是首选治疗，通常已经足够。口服补液盐——在越南任何药店都能买到，商品名 Oresol——将葡萄糖与钠结合，使水分通过肠壁的效率远高于单纯喝白水。正确冲配的几包补液盐，在阴凉处花两三个小时慢慢喝完，通常就能自行解决徒步或海滩活动后的大多数脱水情况。
在较少数的一些情况下，静脉输液才是更好的选择：
- 无法进食进水。持续呕吐意味着口服补液根本没有机会发挥作用。
- 流失速度超过摄入。频繁的水样腹泻，或持续大量出汗，消耗水分的速度比小口补充要快。
- 检查显示确实存在明显脱水——脉搏加快、站立时血压下降、黏膜干燥、尿量很少。
- 由于严重恶心、疲惫或剧烈头痛，身体状况已经无法持续喝水。
静脉输液做的只是绕开肠道，仅此而已。它不含任何神奇成分，也不会给您额外凭空产生的能量。如果评估结果显示只需服用 Oresol 补液盐并休息一个下午，我们会如实推荐这样做。
- 意识混乱、方向感丧失、言语含糊或行为异常
- 晕厥、瘫倒，或无法保持清醒
- 抽搐或癫痫发作
- 体温超过约40°C
- 皮肤灼热干燥，且已经停止出汗
- 连续数小时完全没有排尿
- 无法止住的呕吐，或呕吐物、大便中带血

问诊包含哪些步骤
- 您联系我们，告知酒店或地址、电话号码，以及目前的情况。我们会给您在等待期间可以遵循的建议。
- 先评估。简要询问病史，并进行体检：坐姿和站姿血压、脉搏、体温、血氧饱和度、皮肤和黏膜状况，以及检查上述危险信号。
- 给出并说明决定。如果口服补液是正确的方案，医护人员会告诉您并示范正确的方法。如果需要静脉输液，医护人员会解释原因以及输入的是什么。
- 输液。在手臂或手背的静脉中置入留置针，连接输液管路，根据您的脱水程度设定输液速度。
- 全程监测。医护人员会在整个输液过程中陪伴您，反复检查各项指标，如有变化会及时调整或停止输液。
- 结束处理。拔除留置针并包扎穿刺点，锐器和医疗废物由我们带走，并为您说明当晚需要留意的情况。
一次典型输液包含什么
等渗晶体液——生理盐水（0.9%氯化钠）或乳酸林格液——由医护人员根据您的检查结果和液体流失情况来选择。这些都是全球医院普遍使用的标准补液方案。除非有临床上的必要，例如呕吐已经妨碍了口服摄入时使用的止吐药，否则不会额外添加其他成分；任何药物都由主治医生选择并决定用量。我们不出售维生素混合针剂、谷胱甘肽输液或提升免疫力的输液——没有确切证据表明它们对脱水者能起到水和盐分本身无法起到的作用。
安全与设备
- 治疗由在越南持证执业的医护人员实施。
- 输液袋、留置针、输液器和针头均为工厂密封的一次性用品，会当着您的面拆封。
- 输液前、输液中和输液后都会记录各项指标，全程都有人监测您的状况。
- 锐器和医疗废物由医护团队带走——不会在您的房间留下任何东西。
- 如果您需要医院级别的护理，我们会直接告知并协助安排转诊，而不会不顾情况一味在家中治疗。
